Our 245D Waiver Programs
Jarar Home Healthcare is a 245D licensed provider of respite, homemaker, Individual Community Living Support (ICLS) services, Individual Home Supports (IHS) without training, IHS with training, adult companion services, and night supervision services. For each 245D service, clients select a trusted, qualified worker suited for their needs, while for IHS with training, a qualified staff is matched to the client to meet their needs. In all services, an assigned 245D Jarar HHC coordinator will work with the client and county case manager to set up and administer 245D Waivered Services.

Adult Companion Services (ACS)
Non-medical care, supervision and socialization provided to a person age 18 or older. Adult companion services cover services that help a person work toward a therapeutic or community integration goal in their support plan.

Individual Community Living Supports (ICLS)
Bundled service that includes six service components. ICLS services offer assistance and support for older adults who need reminders, cues, intermittent/moderate supervision or physical assistance to remain in their own homes. ICLS covers assistance and support for eligible people age 65 and older enrolled in the Alternative Care (AC) program or the Elderly Waiver (EW).

Individualized Home Supports (IHS)
Services for people who live in their own home or family home that provide support and/or training in the community living service categories listed in the covered services section.
There are three types of IHS:

Without training.

With training.

With family training.
All types of IHS can be provided in the person’s own home, family’s home or in community spaces used by the general public, and either in person or remotely.

Night Supervision Services
Overnight assistance and supervision provided by a staff person's own home when the person has an assessed need in one of the covered service areas. Night supervision services include an asleep or awake staff in the person’s own home performing an activity, depending on their level of need.

Respite Care
Short-term care services provided to a person when their primary caregiver is absent or needs relief. A person is eligible to receive respite if their primary caregiver(s) are absent or need relief from caregiver duties. Refer to the primary caregiver(s) section for additional information. A person is not eligible to receive respite when they receive any of the services listed in the non-covered services section.
